ABSTRACT BACKGROUND: In Latin America, liver cancer is one of the top causes of cancer mortality. It is the fifth most common cause of death among malignant tumors in Mexico and is the leading cause in Hidalgo State (43.8% of the population living in poverty). OBJECTIVE: To conduct a correlational analysis on the main risk factors for liver cancer in Hidalgo State, Mexico, including municipal disaggregation and comparison with the national level. DESIGN AND SETTING: Cross-sectional, correlational, descriptive and comparative epidemiological study using Mexican governmental databases covering 1990-2019. METHODS: A comprehensive review of the databases of the General Directorate of Health Information (DGIS) was performed to analyze official death figures, hospital discharges and national and municipal population projections, using specific search criteria defined in the Global Burden of Disease classification, based on the risk factors for liver cancer. RESULTS: Liver cancer rates showed an evident rise in Hidalgo (183%), moving from 21st place in Mexico in 1990 to 9th place in 2019. This increase was correlated with alcoholism. An increasing trend for liver cancer deaths, of 133.89%, is projected for 2030. Females and the population over 60 years of age are more affected. There are some critical regions with liver cancer death rates twice the national rate or more. CONCLUSION: Targeted effective public health strategies should be structured by identifying, characterizing and regionalizing critical marginalized municipalities that are vulnerable to alcoholism and other risk factors for liver cancer. This approach may be helpful for other states in Mexico or similar countries.

Reportes actuales sugieren que el antecedente de infección por SARS - CoV-2 y completar un esquema de vacunación otorga mayor protección contra la presentación sintomática de COVID -19. Se comparó el riesgo de enfermar de COVID -19 entre el personal de salud con esquema completo de vacuna contra SARS - CoV-2 BNT162b2 y el antecedente de infección por SARS - CoV-2. Estudio de cohorte histórica en 1874 trabajadores de la salud del Nuevo Hospital Civil de Guadalajara inmunizados con la vacuna BNT162b2 entre enero y marzo de 2021. Después de seis meses de seguimiento, el grupo de no expuestos (sin antecedente de infección) fue de 1397 y el grupo expuesto (con antecedente de infección), de 477 sujetos. La incidencia de infección por SARS - CoV-2 fue de 39 casos. El riesgo de infección en la cohorte posterior a la inmunización fue de 0,021. El grupo de inmunización híbrida presentó un riesgo menor de infección comparado con el grupo de inmunización artificial (0,015 y 0,243). La inmunización híbrida contribuyó a una reducción del riesgo atribuible a la población de 0,003 (R0 0,024; Rp 0,020). La hospitalización se presentó en el 7,69 % de los casos confirmados con SARS - CoV-2. El riesgo de hospitalización en inmunización híbrida es de 0,210 y de 0,143 en el grupo de inmunización artificial (RR 1,46 IC95 % 0,13 -16,11). Se llegó a la conclusión que la inmunización híbrida podría contribuir a reducir el riesgo de infección por SARS - CoV-2, potenciando la inmunidad generada por la vacuna contra COVID -19


Current reports suggest that a history of SARS - CoV-2 infection and completing a vaccination schedule provides greater protection against the symptomatic presentation of COVID -19. The risk of becoming ill with COVID -19 was compared between health personnel with a complete SARS - CoV-2 BNT162b2 vaccine schedule and a history of SARS - CoV-2 infection. Historical cohort study in 1874 health workers of the New Civil Hospital of Guadalajara immunized with the BNT162b2 vaccine between January and March 2021. After six months of follow-up, the non-exposed group (without a history of infection) was 1397 and the exposed group (with a history of infection), of 477 subjects. The incidence of SARS - CoV-2 infection was 39 cases. The risk of infection in the post-immunization cohort was 0.021. The hybrid immunization group had a lower risk of infection compared to the artificial immunization group (0.015 and 0.243). Hybrid immunization contributed to a population-attributable risk reduction of 0.003 (R0 0.024, Rp 0.020). Hospitalization occurred in 7.69% of confirmed cases with SARS - CoV-2. The risk of hospitalization in hybrid immunization is 0.210 and 0.143 in the artificial immunization group (RR 1.46 CI95% 0.13 -16.11). It was concluded that hybrid immunization could help reduce the risk of SARS - CoV-2 infection, enhancing the immunity generated by the vaccine against COVID -19

ABSTRACT

O objetivo do estudo foi analisar a composição corporal com utilização do DXA e correlacioná-la com a idade cronológica em adolescentes pós-púberes, de ambos os sexos. Participaram da pesquisa 46 adolescentes em fase pós-puberal, sendo 27 meninas (17,23±0,98) e 19 meninos (17,65±0,74) de ensino médio de uma escola estadual da Zona Sul da cidade do Rio de Janeiro. Foram realizadas uma anamnese, avaliação antropométrica e o DXA para avaliações. O teste U de Mann Whitney e o coeficiente de correlação de Spearman foram utilizados, adotando P<0,05 para significância. Verificou-se que as meninas apresentam composição corporal diferentedos meninos (P<0,05) e que com o aumento da idade elas tendem a diminuir o percentual de gordura, embora tenha sido observado um quantitativo alto de meninas com percentual de gordura elevado. Os meninos apresentaram uma tendência de aumento do percentual de gordura e da massa magra com o aumento da idade. As medidas de conteúdo e densidade mineral ósseo dentro da normalidade, com tendência de aumento com o avanço da idade cronológica. Conclui-se que os grupos masculino e feminino apresentaram comportamentos diferenciados quanto à composição corporal e os valores apresentados trazem mais um complemento à literatura a respeito de referências para a composição corporal, obtida com o DXA, em adolescentes pós-púberes.


The purpose of this study was to analyze body composition using DXA and correlate it with chronological age in post-pubertal adolescents of both genders. A total of 46 adolescents participated in the study, of which 27 were girls (17.23±0.98) and 19 boys (17.65±0.74) from a state school in the South District in the city of Rio de Janeiro. An anamnesis, anthropometric evaluation and DXA were performed for evaluations. The Mann-Whitney U test and the Spearman correlation coefficient were used, adopting P <0.05 for significance. Girls were found to have a different body composition than boys (P <0.05) and that, with increasing age, they tended to present a decrease in fat percentage, although in general a high number of girls presented a high fat level. The boys presented a tendency to increase the percentage of fat and lean mass with the increase of the age. Measurements of bone mineral content and density were considered as being within normality, with a tendency to increase with the advancing of the chronological age. Girls and boys present a different behavior regarding body composition and the values presented in this study bring an addition to the literature regarding body composition references through DXA in post-pubertal adolescents.

ABSTRACT

BACKGROUND: Chagas disease is considered important and presents intense inflammatory and fibrotic processes induced by the perpetuation of the parasite in the affected tissues and organs. Therefore, it is necessary to inquire about the host defense and attack mechanisms to have a more detailed knowledge about Chagas disease. Micro-RNAs are found in blood, tissues and extracellular vesicles. These small regulators of gene expression are involved in physiological and pathological processes in both mammals and parasites. Several microRNAs have deregulated expression in chagasic heart disease, although little is known about their extracellular expression. Our main objective was to evaluate the involvement of miR-21, miR-146a and miR-155 in several samples from mice infected with the TcI Ninoa strain from the acute and indeterminate phases. We also explored a potential functional association of the selected microRNAs using STRING software. This software identified 23 pathways associated with Trypanosoma cruzi infection. In addition, eleven genes were identified through bioinformatics analysis, and we found that SMAD family member 5 was downregulated in both phases. This gene serves as a mediator in the TGF-ß signaling pathway. Thus, forty female mice of the CD1 strain were distributed into 4 groups and the expression levels of miR-21, miR-146a and miR-155 were measured in samples of heart tissue, total plasma and plasma extracellular vesicles by quantitative real-time polymerase chain reaction. RESULTS: Overexpression of miR-21, miR-146a and miR-155 was observed in heart and plasma in both phases. Moreover, in extracellular vesicles miR-21 and miR-146a were also overexpressed in the acute phase, whereas in the indeterminate chronic phase we found only miR-146a up-regulated. CONCLUSIONS: The expression of inflammatory microRNAs miR-21, miR-146a and miR-155 were up-regulated in each of the samples from acutely and chronically infected mice. The relevant finding was that miR-146a was up-regulated in each sample in both phases; therefore, this miRNA could be a possible candidate biomarker in Chagas disease.

Resumen Objetivo: Resumir la evidencia científica sobre las alteraciones renales asociadas con la infección por SARS-CoV-2. Material y métodos: Se realizó una revisión rápida con la metodología Cochrane. Resultados: La enfermedad renal crónica (ERC) preexistente en pacientes con SARS-CoV-2 varió de 1 a 38% y la lesión renal aguda (LRA), de 2.9 a 86.4%. El pronóstico de la infección fue peor en pacientes con ERC y en aquellos con reserva renal remanente (RRR) intacta que desarrollaron LRA. El riesgo de muerte fue mayor (riesgo relativo combinado = 1.49; IC95%: 1.09-2.04) en pacientes infectados por SARS-CoV-2 con ERC preexistente. Los marcadores de RRR mostraron alteraciones en pacientes con SARS-CoV-2 graves y fatales; el marcador más utilizado fue la creatinina sérica. Conclusiones: La evidencia científica muestra la relevancia de la evaluación y monitoreo permanente de la RRR en pacientes hospitalizados por SARS-CoV-2 para mejorar el pronóstico de aquellos con ERC preexistente, así como de aquellos sin ERC que desarrollan LRA.


Abstract: Objective: To summarize the scientific literature on kidney abnormalities associated with SARS-CoV-2 infection. Materials and methods: It was conducted a rapid review using the Cochrane methodology. Results: Pre-existing chronic kidney disease (CKD) in SARS-CoV-2 infected patients ranged from 1-38% and acute kidney injury (AKI) ranged from 2.9-86.4%. The prognosis of patients with SARS-CoV-2 infection was worse among those with CKD and those with normal remnant kidney function (RKF) that developed AKI. The risk of death was higher (pooled risk ratio =1.49; 95%CI: 1.09-2.04) among SARS-CoV-2 infected patients with pre-existing CKD. The RKF markers showed alterations among severe and non-surviving SARS-CoV-2 patients; the most common marker was serum creatinine. Conclusions: The scientific evidence shows the relevance of the evaluation and permanent monitoring of the RKF in SARS-CoV-2 hospitalized patients to improve the prognosis of those with pre-existing CKD as well as the prognosis of those without CKD who develop AKI.

ABSTRACT

Resumen El linfoma de Hodgkin (LH) se debe a la transformación clonal de células originadas en los linfocitos B, lo que genera las células binucleadas patognomónicas de Reed-Sternberg. El LH es una enfermedad de células B con una distribución bimodal, con mayor incidencia en la adolescencia y la tercera década de la vida y un segundo pico en personas mayores de 55 años. Las células del LH clásico habitualmente sufren una reprogramación de la expresión génica, ya que pierden la expresión de la mayoría de los genes típicos de las células B y han adquirido la expresión de múltiples genes que son típicos de otros tipos de células del sistema inmunitario. El algoritmo de tratamiento dependerá si se trata de LH clásico o de predominio linfocítico, si es un estadio temprano con marcadores de pronóstico desfavorables o no, el esquema inicial de manejo y si existe enfermedad voluminosa, entre las variables más relevantes.


Abstract Hodgkin's lymphoma is due to the clonal transformation of cells originating from B lymphocytes, generating the pathognomonic binucleate Reed-Sternberg cells. Hodgkin's lymphoma is a B cell disease with a bimodal distribution, with higher incidence in adolescence and the third decade of life, showing a second peak in people over 55 years of age. Classic Hodgkin lymphoma cells routinely undergo gene expression reprogramming, as they lose the expression of most of the typical B-cell genes and acquire the expression of multiple genes that are typical of other types of cells in the immune system. The treatment algorithm will depend on whether it is classic or predominantly lymphocytic HL, if it is early stage with unfavorable prognostic markers or not, the initial management regimen, and whether there is bulky disease, among the most relevant variables.

Abstract: The purpose of this study was to describe the synthesis, characterization, and functionalization of b-NaYF4:30%Yb/0.5%Tm upconverting nanocrystals for use as nanofillers in a dental adhesive and microscopically evaluate the interface between the particles and a commercial adhesive. The upconverting nanoparticles were synthesized and purified by thermal decomposition, and their chemical composition determined by energy dispersive X-Ray spectroscopy. The crystalline structure was characterized using X-Ray diffraction and morphology and size were observed with scanning and transmission electron microscopy. Upconverting emission was evaluated by spectrophotometry irradiating the particles with a 975 nm diode laser. Particles were functionalized with polyacrylic acid and the success was confirmed by measurement of Zeta Potential and transmission electron microscopy. The results of X-ray diffraction found a pure hexagonal phase crystalline pattern. Scanning electron microscopy showed uniform dispersion of hexagonal-shaped particles of approximately 150 nm. Upconversion emission was observed in 344 nm, 361 nm, 450 nm, 474nm, 646 nm, 803 nm. Functionalization success was confirmed by formation of a stable aqueous colloid with a Zeta potential of −29.5mV and the absence of voids in the particle-adhesive interface on the transmission electron microscopy images. The reported synthesis and functionalization process produced upconverting nanoparticles emitting photons within the blue spectral region (450 nm and 474 nm).

Resumen: Objetivo: Examinar prospectivamente los predictores de alteraciones subclínicas de la función renal en adultos sin diagnóstico previo de enfermedad renal crónica. Material y métodos: Se analizaron datos de una cohorte de adultos mexicanos (n=757). La función renal (2010) se midió mediante la tasa de filtración glomerular estimada (TFGE-Cr), creatinina y ácido úrico séricos. Los predictores (2004) se identificaron con modelos de regresión lineal y logística. Resultados: Se clasificó 33% con TFGE-Cr disminuida. La TFGE-Cr fue menor en hombres, en >40 años y en usuarios de antihipertensivos; y fue mayor en aquellos con consumo proteico alto. Los predictores de la creatinina fueron similares a los de la TFGE-Cr. El ácido úrico disminuyó en participantes hombres, con obesidad, hipertensión e hipercolesterolemia. Conclusión. Los biomarcadores séricos convencionales son útiles para identificar alteraciones subclínicas de la función renal. Algunos predictores de la función renal son potencialmente modificables, por tanto susceptibles de intervención.


Abstract: Objectives: Our aim was to examine prospectively predictors of subclinical renal alterations among adults without chronic kidney disease. Materials and methods: We analyzed data from a cohort of Mexican adults (n=757). Kidney function (2010) was assessed with the estimated glomerular filtration rate (eGFR-Cr), serum creatinine and uric acid. Predictors of each kidney function marker (2004) were identified with linear and logistic regression models. Results: 33% had an eGFR-Cr <90 ml/min/1.73 m2. eGFR-Cr was lower among men, those >40 years old and users of antihypertensive medication; and higher among those with a high protein intake. Serum creatinine predictors were similar to those observed for eGFR-Cr. Uric acid decreased in men, among those with obesity, hypertension and high cholesterol. Conclusion: Conventional kidney function biomarkers are useful to identify subclinical alterations. Some predictors of kidney function are potentially modifiable, therefore susceptible for intervention among high-risk groups.

ABSTRACT

Objective: analyze the agreement of the V̇O2max values estimated by American College of Sports Medicine and Foster equations with direct measure gas analyze in young Brazilian males. The maximal oxygen uptake, as a health indicator and mortality predictor, can be assessed in different ways. The gold standard comprises the direct measurement of exhaled gases, which entails high cost. A more conveniently form can be estimation equations. Materials and methods: this study assessed VO2max of 41 young Brazilian males (21.4 ±2.2 years) by cardiopulmonary exercise test in a treadmill ergometer with a ramp protocol. Bland and Altman analysis was performed to verify the agreement between V̇ O2max measured and estimated values by ACSM and Foster equations. Results:the measured VO2max was 52.3 ± 4.9 ml.kg-1.min-1. The difference between the measured V̇O2max and the estimated V̇ O2max by the ACSM equation (9.40±3.67) was approximately 7.5 times greater than the difference between the measured V̇O2max and estimated V̇O2max by Foster's equation (1.25±3.46). Bland Altman graphics shows that only ACSM equation had mean differences that were significantly different from the measured value. Conclusions: the ACSM equation showed not appropriate for during treadmill stress testing young adults in a ramp protocol and Foster equation seems to be a more accurate estimator of V̇O2max for this population, besides showed a bias along the aerobic capacity, trending to overestimates and underestimates V̇O2max of least and most fit people, respectively..(AU)


Objetivo: analizar la concordancia de los valores de VO2max estimados por las ecuaciones del Colegio Americano de Medicina del Deporte y de Foster con el análisis de gases de medida directa en varones brasileños jóvenes. el consumo máximo de oxigeno, como un indicador de salud y predictor de mortalidad, se puede evaluar de diferentes maneras. El estándar de oro comprende la medición directa de los gases exhalados, lo que implica un alto costo. Una forma más conveniente puede ser las ecuaciones de estimación. Materiales y métodos: este estudio evaluó el VO2máx de 41 hombres brasileños jóvenes (21,4 ± 2,2 años) mediante una prueba de ejercicio cardiopulmonar en un ergómetro en cinta ergométrica con un protocolo de rampa. El análisis de Bland y Altman se realizó para verificar la concordancia entre V̇O2max medido y valores estimados por las ecuaciones del ACSM y de Foster. Resultados: el VO2max medido fue de 52,3 ± 4,9 ml.kg-1.min-1. La diferencia entre el V̇O2max medido y el VO2max estimado por la ecuación ACSM (9,40 ± 3,67) fue aproximadamente 7.5 veces mayor que la diferencia entre el VO2max medido y el VO2max estimado por la ecuación de Foster (1,25 ± 3,46). Los gráficos de Bland Altman muestran que solo la ecuación de ACSM tenía diferencias estadísticas del valor medido. Conclusiones: la ecuación ACSM no fue adecuada durante la prueba de ejercicio en cinta de correr en adultos jóvenes en un protocolo de rampa y la ecuación de Foster parece ser un estimador más preciso de VO2max para esta población, además mostró un sesgo a lo largo de la capacidad aeróbica, con tendencia a sobreestimar y subestimar VO2 máx. de personas menos y más en preparadas, respectivamente..(AU)

Abstract Aims: Recently, high-intensity training methods have become popular, integrating the cardiovascular and neuromuscular training in a single training session, among these methods is CrossFit®. The objective of this study was to analyze the superficial thermal response to CrossFit® exercise in men and women, in order to use this knowledge to prevent overuse injuries. Methods: Nineteen volunteers involved in CrossFit® exercise for more than 6-month (12 males and 7 females) were recruited. The acquisition of the thermal images was performed in a climatized room in two moments, at rest (before exercise), and after one CrossFit® training session. The training session lasted 45min, comprising warm-up (10-min), accessory work (15-20min), and workout of the day (15-20-min). Before the first image acquisition, volunteers were acclimated for 15 min. The Wilcoxon signed-rank test was used to compare the skin temperature between pre- and post-exercise. Results: Temperatures rose significantly pre- to post-exercise in the forearm and anterior thigh regions, while it decreased in the anterior thorax and dorsal lower back regions. These results were found both, in the overall sample, and the male volunteers, but not when the female results were isolated. Conclusion: It can be concluded that superficial thermal response to one CrossFit® training session was characterized and was different for men and women. The superficial thermal responses were aligned with the physiological alterations promoted by other modalities, such as resistance training, cycling, and running.

ABSTRACT

The eclectus parrot (Eclectus roratus) is a species of parrot native to the Solomon Islands, Sumba, New Guinea and nearby islands, northeastern Australia, and the Moluccan Islands. The wide range of behaviors emphasizes the existence of knowledge of only a mere fraction of the significant behavioral parameters of these birds. The objective of this study is to identify the behavior of the eclectus parrot kept in captivity and to check the frequency of these birds in certain places of the nursery, comparing the results to other studies about parrots and the behavior in nature. This is a study about the behavior of a bird with a very pronounced sexual dimorphism, where the males have a predominantly green color, and the females are mainly red-colored. In the wild, while the male perches on the outer branches of the trees, thefemale usually perches close to tree trunks. This location, together with her red and blue plumage, provides the ideal camouflage in the dark interior of the trees.

Resumen: Objetivo: Conocer las características de la educación médica e identificar sus fortalezas y debilidades. Material y métodos: Se realizó un estudio transversal y cuantitativo para conocer las características de la educación médica en 29 escuelas de medicina en México, entre abril y septiembre de 2017. Se utilizó un cuestionario con escala tipo Likert para explorar el contexto, la regulación, la estructura, el proceso, los resultados y el impacto de la educación médica. Se realizó un análisis bivariado con ji cuadrada y una significancia estadística depigual o menor a 0.05. Resultados: El contexto político obtuvo 64%, el contexto económico 10%, los mecanismos de regulación 31%, la estructura educativa 61% y el impacto social 93%. Conclusiones: Se requiere fortalecer las políticas públicas, la regulación y la inversión pública, para mejorar la calidad de la educación médica.


Abstract: Objective: To know the characteristics of medical education and identify its strengths and weaknesses. Materials and methods: A transversal and quantitative study of the characteristics of medical education in 29 medical schools in Mexico was carried out, between April and September 2017. Questionnaire with Likert scale was applied to explore context, regulation, structure, process, results and impact of medical education. Bivariate analysis was performed with a Chi square test and the significance level was equal to or less than 0.05. Results: The political context obtained 64%, economical context 10% and mechanisms of regulation 31%. The educational structure was 61% and the social impact was 93%. Conclusions: Public policies, regulatory mechanisms and public investment must be strengthened to improve the quality of medical education.

ABSTRACT

Introdución: las necesidades y condiciones socioeconómicas en México han cambiado, así como el modelo de salud ha modificado las competencias de los profesionales de salud. Enfermería asume la prescripción de medicamentos como una nueva competencia. Cabe señalar que esta ya se realizaba, sólo que sin sustento legal. Objetivo: conocer la percepción y el nivel de conocimientos farmacológicos de los alumnos de licenciatura en enfermería para prescribir medicamentos. Material y métodos: se realizó un estudio transversal, la unidad de análisis fueron alumnos de licenciatura de enfermeria que cursaban farmacología, la muestra fue de 160 estudiantes. Se utilizó un cuestionario web, que contemplaba 6 módulos correspondíentes a las unidades del programa de la asignatura y para conocer la percepción de la prescripción se aplicó una entrevista semiestructurada. Resultados: el promedio general de conocimientos farmacológicos de los alumnos fue insuficiente. Ellos perciben que no están del todo cualificados, para prescribir medicamentos de acuerdo con la normatividad vigente; principalmente por la falta de conocimientos farmacológicos. Conclusiones: la prescripción de medicamentos, es un proceso que se encuentra en evolución en el país. Una vez que se consolide, tendrá un impacto en el sistema de salud, al igual que, en la calidad de los cuidados que brinde enfermería.

ABSTRACT

The incidence of musculoskeletal injuries (MSIs) is a constant concern in all Armed Forces. Among risk factors for MSIs, physical training and sports can be considered the most frequent cause in military. In this sense, the aim of this study was to investigate the incidence rate of musculoskeletal injuries, identifying the anatomical distribution and its association with risk factors such as: body composition, age and physical training over the past 12 months. Three hundred fifty-one male cadets from the first year of the Brazilian Army officers training course and four hundred and fifty six students from the logistics sergeants training course (three hundred seventy-five male and eighty one female) volunteered to fill in a self-reported questionnaire which was developed based on a literature review and the clinical experience of the investigators. It contained questions concerning anatomical site of the MSIs, hours of physical activities per week, type of activities at the moment of injury and days of absences in physical activities. Among 807 subjects studied, 180 military have reported 220 MSIs. From this total, 143 have presented a single injury, 34 have suffered two injuries and three subjects have had three injuries each one during the last 12 months. Knee was the anatomical site with the higher incidence (7.06%) of MSIs over a 12-month period. Running was the activity with the higher incidence (11.77%) of injuries over a 12-month, accounting for 43.18% of all MSIs. When considered all activities of physical training, the MSIs incidence rate over a 12-month was 17.97%, which represents 71.36% of all MSIs. In young militaries, the largest amount musculoskeletal injuries occurred in the lower extremities and during military physical training, with running being the main activity in which the injuries occurred. Population, gender and body composition was not significantly associated with these types of injuries, while older age was associated in this study. The gradual and systematic progression of distance and speed during running training should be emphasized to prevent MSIs. (AU)


A incidência de lesões musculoesqueléticas (LME) é uma preocupação constante em todas as Forças Armadas. Entre os fatores de risco para LME, treinamentos físicos e esportes podem ser considerados a causa mais frequente em militares. O objetivo deste estudo foi investigar a taxa de incidência de lesões musculoesqueléticas, identificar a distribuição anatômica e sua associação com fatores de risco, tais como: composição corporal, idade e treinamento físico nos últimos 12 meses. Trezentos e cinquenta e um cadetes do primeiro ano da Escola Preparatória de Cadetes do Exército brasileiro e quatrocentos e cinquenta e seis alunos do curso de Sargento logístico (trezentos e setenta e cinco homens e oitenta e uma mulheres) se ofereceram para preencher um questionário auto relatado que foi desenvolvido com base em uma revisão da literatura e a experiência clínica dos pesquisadores. Ele continha perguntas sobre o local anatômico das LME, horas de atividades físicas por semana, tipo de atividades no momento da lesão e dias de ausência em atividades físicas. Entre 807 sujeitos estudados, 180 militares relataram 220 LME. A partir deste total, 143 apresentaram uma única lesão, 34 sofreram duas lesões e três indivíduos tiveram três lesões cada nos últimos 12 meses. O joelho foi o local anatômico com maior incidência (7,06%) de LME ao longo de um período de 12 meses. A corrida foi a atividade com maior incidência (11,77%) de lesões ao longo de 12 meses, representando 43,18% de todas as LME. Quando considerado todas as atividades de treinamento físico, a taxa de incidência de MSI ao longo de um período de 12 meses foi de 17,97%, o que representa 71,36% de todas as LME. Em militares jovens, a maior quantidade de lesões musculoesqueléticas ocorreu nas extremidades inferiores e durante o treinamento físico militar, sendo a corrida a principal atividade na qual ocorreram as lesões. População, gênero e composição corporal não foram significativamente associados a esses tipos de lesões, enquanto a idade avançada foi associada neste estudo. A progressão gradual e sistemática da distância e da velocidade durante o treinamento em corrida deve ser enfatizada para evitar LME. (AU)

ABSTRACT

The combat simulation exercise is a military activity where combat activities characterized by intense physical activity are simulated. The aim of this study was to describe the behavior of indirect markers of muscle damage during military activities of combat simulation with the realization of strenuous physical efforts. This study was conducted with military recruits/volunteers (n=43; 19-24 age group) from the COMANDOS Army Corporal Training Course. The biomarkers were evaluated at the baseline (T0), 72 hours after the baseline (T1) and 63 hours after the end of the activity (T2). To compare the variables analyzed at different times, an analysis of variance (ANOVA) was applied to repeated measures with post hoc Tukey's test to identify the statistically significant differences. The criteria of significance adopted was the value of p<0,05. On average, the total body mass was significantly greater in T0 when compared to T1 (74,4kg x 69,8Kg; p< 0,05). In relation to the biochemical markers, on average, the serum levels were significantly higher (p<0,001) in T1 when compared to T0: CPK, LDH, CRP, Mb and AGPA. After 63 hours of rest, on average, the serum levels of the biochemical markers were significantly lower than in the moment T1: CPK, LDH, CRP, Mb and AGPA. However, after 63 hours of the end of the activity, all variables evaluated returned to baseline, with the exception of LDH, that on an average remained above the values observed in the moment T0. This study made possible to understand the behavior of indirect markers of muscle damage during the COMANDOS Army Corporal Training Course. However, others studies are necessary, regarding to Training Courses such as: paratrooper, mountain, jungle warfare and others to try establish a possible range of reference values for this markers during military activities.


O exercício de simulação de combate é uma atividade militar, onde as atividades de combate caracterizadas por intensa atividade física são simuladas. O objetivo deste estudo foi descrever o comportamento dos marcadores indiretos de lesão muscular durante as atividades militares de simulação de combate com a realização de esforços físicos extenuantes. Este estudo foi realizado com militares voluntários (n = 43; faixa etária = 19-24 anos) do Curso de Formação de Cabos do Exército. Os biomarcadores foram avaliados na linha de base (T0), 72 horas após o início das atividades (T1) e 63 horas após o final da atividade de (T2). Uma análise de variância (ANOVA) para medidas repetidas, com teste post hoc Tukey, foi aplicada para identificar as diferenças estatisticamente significativas nas variáveis analisadas em momentos diferentes. O critério de significância adotado foi o valor de p <0,05. Em média, a massa corporal total foi significativamente maior no T0 quando comparado a T1 (74,4kg x 69,8Kg; p <0,05). Em relação aos marcadores bioquímicos, em média, os níveis séricos foram significativamente maiores (p <0,001) em T1 quando comparados com T0 para: CPK, LDH, CRP, Mb e AGPA. Após 63 horas de descanso, em média, os níveis séricos dos marcadores bioquímicos (CPK, LDH, CRP, Mb e AGPA ) foram significativamente menores do que no momento T1. No entanto, após 63 horas a partir do fim da atividade, todas as variáveis avaliadas retornaram à linha de base, com a exceção do LDH, que, em média, manteve-se significativamente maior do que os valores observados no momento T0. Este estudo possibilitou compreender o comportamento dos marcadores indiretos de lesão muscular durante o Curso de Formação de Cabos COMANDOS. No entanto, outros estudos são necessários, com relação a cursos de formação, tais como: paraquedistas, montanha, guerra na selva, e outros, para tentar estabelecer uma possível faixa de valores de referência para estes marcadores durante as atividades militares.

ABSTRACT Introduction: Since 1965, the Brazilian Army (BA) has participated in peacekeeping missions of the United Nations (UN), sending large contingents to various parts of the world, such as Angola, Mozambique, East Timor and Haiti. To do so, the military must be in a minimum physical condition to face any obstacles in the course of action in the theater of operations. Objective: To verify the effects of Military Physical Training (MPT) on muscular strength of upper limbs, anthropometric variables and body composition of Brazilian military personnel of the 24th UN peacekeeping mission in Haiti. Methods: The sample consisted of 192 male soldiers with mean age of 23.2 ± 3 6 years. They observed a weekday residential regime, with a defined schedule of meals and activities and time off on weekends. The MPT was applied four times a week, 90 minutes long for 14 weeks and involved aerobic and resisted exercises. In the pre- and post-MPT period, body mass (MC), height, waist circumference (WC) and abdomen circumference (AC) were measured to calculate body mass index (BMI) and fat percentage (%F) through the protocol of Salem.. In addition, 12-minute run tests, push-ups, pull up and abdominal flexions were performed to calculate the Rogers Index (RI). Results: When comparing pre- and post-MPT, a significant difference was observed in all anthropometric variables and body composition (BM, BMI and %F) and in the RI for the upper limb muscle strength. Conclusion: The 14-week MPT indicates an improvement in the anthropometric and body composition variables, as well as an increase in upper limb muscular strength in Brazilian soldiers of the UN peacekeeping contingents in Haiti. This study concluded that RI could be an effective tool for assessing military muscular strength. Level of Evidence II; Therapeutic studies - Investigating the results of treatment.


RESUMO Introdução: Desde 1965, o Exército Brasileiro (EB) participa das missões de paz da Organização das Nações Unidas (ONU) enviando grandes contingentes para diversos lugares do mundo, como Angola, Moçambique, Timor Leste e Haiti. Para isso, os militares precisam estar em condições físicas mínimas para enfrentar quaisquer obstáculos no decorrer das ações no teatro de operações. Objetivo: Verificar os efeitos do Treinamento Físico Militar (TFM) sobre a força muscular dos membros superiores, as variáveis antropométricas e a composição corporal de militares brasileiros integrantes do 24º contingente de missão de paz da ONU no Haiti. Métodos: A amostra foi constituída de 192 militares do sexo masculino com média de idade de 23,2 ± 3,6 anos. Eles cumpriram regime de internato, tendo atividades e refeições com horários definidos, sendo liberados nos finais de semana. O TFM foi aplicado quatro vezes por semana, com 90 minutos de duração por 14 semanas e envolveu exercícios aeróbicos e resistidos. No período pré e pós-TFM foram medidas massa corporal (MC), altura, circunferência da cintura (CC) e do abdome (CA) para calcular o índice de massa corporal (IMC) e o percentual de gordura (%G) com o protocolo de Salem. Além disso, foram realizados testes de corrida de 12 minutos, flexão de braço, abdominal supra e flexão na barra para calcular o índice de Rogers (IR). Resultados: Ao comparar os resultados pré e pós-TFM verificou-se diferença significativa em todas variáveis antropométricas e composição corporal (MC, IMC e %G) e também no IR para força muscular dos membros superiores. Conclusão: O TFM em 14 semanas aponta melhora nas variáveis antropométricas e de composição corporal, bem como aumento da força muscular dos membros superiores em militares brasileiros integrantes dos contingentes de missões de paz da ONU no Haiti. Este estudo concluiu que o IR pode ser uma ferramenta eficaz para avaliação de força muscular de militares. Nível de Evidência II; Estudos terapêuticos - Investigação dos resultados do tratamento.

Para ello, los militares necesitan estar en condiciones físicas para hacer frente a cualquier obstáculo en el curso de las acciones en el teatro de operaciones. Objetivo: Observar los efectos del Entrenamiento Físico Militar (EFM) sobre la fuerza muscular de las extremidades superiores, las variables antropométricas y la composición corporal de los miembros brasileños del 24º Contingente de Misión de Paz de la ONU en Haití. Métodos: La muestra fue constituida por 192 militares del sexo masculino con promedio de edad de 23,2 ± 3,6 años. Ellos cumplieron régimen de internado con actividades y comidas en horarios definidos, siendo liberados los fines de semana. El EFM se aplicó cuatro veces por semana, con 90 minutos de duración durante 14 semanas e involucró ejercicios aeróbicos y resistidos. En los periodos pre y post-EFM fueron medidas masa corporal (MC), altura, circunferencia de la cintura (CC), del abdomen (CA), para calcular el índice de masa corporal (IMC) y el porcentaje de grasa corporal (%G) con el protocolo de Salem. Además, se realizaron pruebas de carrera de 12 minutos, flexión de brazo, abdominal supra y flexión en la barra para calcular el Índice de Rogers (IR). Resultados: Al comparar los resultados pre y post-EFM se observó diferencia significativa en todas las variables antropométricas y de composición corporal (MC, IMC, %G) y también en el IR para fuerza muscular de las extremidades superiores. Conclusión: El EFM en 14 semanas muestra mejora en las variables antropométricas y de composición corporal, así como aumenta la fuerza muscular de las extremidades superiores en militares brasileños integrantes de los contingentes de misiones de paz de la ONU en Haití. Este estudio concluyó que el IR puede ser una herramienta eficaz para evaluar la fuerza muscular de los militares. O constante aprimoramento dos materiais odontológicos é impulsionado pelas pesquisas científicas que, por sua vez,são incentivadas pelo mercado consumidor que busca tratamentos estéticos e duráveis. Objetivo: avaliar a resistência flexuralde duas resinas compostas convencionais e uma resina composta do sistema bulkfill. Metodologia: foram utilizadas as resinas compostas: Z100TM (microhíbrida), FiltekTM Z350 XT (nanoparticulada) e a FiltekTM Bulk fill (sistema bulkfill). Foram confeccionados dez corpos de prova de cada compósito (25x2x2mm). Testes mecânicos para avaliação da resistência flexural foram realizados, os valores de força obtidos para fratura do corpo de prova foram, então, transformados em MPa. Resultados: o valor médio de resistência flexural médio encontrado para o grupo Z100TM=108,43MPa, para o grupo FiltekTM Bulk Fill=133,30MPa e para o grupo FiltekTM XT Z350=172,53MPa. Conclusões: a resina composta convencional denanoparticulaFiltekTM XT Z350 apresentou os maiores valores de resistência flexural, seguida da resina FiltekTMBulk fill. A resina Z100TM apresentou os menores valores médios de resistência flexural.


The constant improvement of dental materials is driven by scientific research which, in turn, is encouraged by the consumer market seeking aesthetic and durable treatments.Aim: to evaluate the flexural strength between two conventional composite resins and a composite resin of the bulk fill system. Methodology: composite resins were used: Z100TM (microhybrid), FiltekTM XT Z350 (nanoparticulate) and FiltekTM Bulk fill (bulk fill system). Ten specimens of each composite (25X2X2mm) were made. Mechanical tests for evaluation of flexural strength were performed, strength values obtained for fracture of the specimen were, then, transformed into MPa.Results: the mean value for the Z100TM group = 108.43MPa, for the FiltekTM Bulk Fill group = 133.30MPa and the FiltekTM XT Z350 group = 172.53MPa. Conclusions: the conventional composite resin of FiltekTM XT Z350 showed the highest values of flexural strength, followed by FiltekTM Bulk fill resin. Z100TM resin showed the lowest values of flexural strength.

Abstract: Objective: To examine different health outcomes that are associated with specific lifestyle and genetic factors. Materials and methods: From March 2004 to April 2006, a sample of employees from three different health and academic institutions, as well as their family members, were enrolled in the study after providing informed consent. At baseline and follow-up (2010-2013), participants completed a self-administered questionnaire, a physical examination, and provided blood samples. Results: A total of 10 729 participants aged 6 to 94 years were recruited at baseline. Of these, 70% were females, and 50% were from the Mexican Social Security Institute. Nearly 42% of the adults in the sample were overweight, while 20% were obese. Conclusion: Our study can offer new insights into disease mechanisms and prevention through the analysis of risk factor information in a large sample of Mexicans.


Resumen: Objetivo: Examinar diferentes desenlaces en salud y su asociación con factores genéticos y del estilo de vida. Material y métodos: De marzo de 2004 a abril de 2006, una muestra de empleados de tres diferentes instituciones de salud y académicas, así como miembros de sus familias, fueron enrolados en el estudio, previa firma de consentimiento informado. Durante la medición basal y el seguimiento (2010-2013) los participantes completaron un cuestionario autoaplicado, exámenes físicos y proporcionaron muestras sanguíneas. Resultados: Fueron incluidos participantes (10 729) de entre 6 y 94 años en la medición basal. De estos, 70% fueron mujeres y 50% del Instituto Mexicano del Seguro Social. Aproximadamente 42% de los adultos tuvieron sobrepeso y 20% obesidad. Conclusión: Este estudio puede ofrecer conocimientos sobre los mecanismos de la enfermedad a través del análisis de factores de riesgo en una muestra de mexicanos.
